Clinical Lead & First Hire

New York, NYPart-time → Full-timeEarly-stage startupEquityNo TMS experience required

Build a new model for brain health from the ground up — and own a piece of what you build.

About Pharia

Pharia helps people access more of their mind’s capacity — improving focus, presence, motivation, and emotional steadiness — to improve their mental performance & wellbeing.

We’ve adapted a 20-year-old, FDA-cleared technology — transcranial magnetic stimulation (TMS) — for high-functioning individuals who want more control over how they think, feel, and perform. Our treatments are personalized and guided by world-class physicians and specialists.

We’re early-stage and have treated dozens of high performers (founders, executives, and others) to date. Our founding team includes physician-scientists and clinicians with nearly a decade of brain stimulation experience.

We’re looking for someone who’s excited to shape how Pharia gets experienced. We’ll train you on what you need to know, and you’ll dive in quickly.

The Role

First hire. Full ownership.

This is a rare first-hire opportunity at an early-stage healthcare startup. You’ll begin as our boots-on-the-ground clinician — assessing patients, running treatment sessions, building clinical workflows. Over time, this role evolves into Clinical Director or Head of Operations, depending on where your strengths take you.

No prior TMS experience required. You’ll train directly under one of the country’s top neuropsychiatrists, with proprietary software tools supporting your clinical decision-making every step of the way.

First 90 days

  • Conduct patient intakes and assessments for TMS eligibility
  • Administer and monitor accelerated TMS sessions with full training and supervisory support
  • Build and refine clinical SOPs for our first permanent New York location
  • Serve as the primary patient-facing presence — setting the tone for the Pharia experience
  • Work closely with the founding team on scheduling, patient flow, and operations

As you scale — 6 to 18 months

  • Take ownership of clinical operations — staffing, scheduling, quality, compliance
  • Train and supervise incoming clinical staff as the team grows
  • Contribute to referral partnerships with PCPs, psychiatrists, DPC practices, and longevity clinics
  • Help design the clinic blueprint we replicate as we expand to other markets
  • Grow into a Clinical Director or VP of Operations role with meaningful equity

Who We’re Looking For

Builder mindset. Clinical excellence.

The right person has probably felt constrained by traditional clinical environments. You noticed inefficiencies and wanted to fix them. You connected with patients in a way that felt more like a relationship than a transaction. You had ideas about how care should work — and didn’t have space to try them.

Required

  • Active New York NP license — PMHNP required; FNP or AGNP with substantial psychiatric or mental health clinical experience considered
  • Prescriptive authority under a Collaborative Practice Agreement, or ability to obtain
  • Genuine interest in mental health, cognitive performance, and human potential
  • Entrepreneurial track record — you’ve built something, run something, or worked somewhere you had real ownership (DPC, concierge, wellness clinic, etc.)
  • Exceptional interpersonal presence — this patient population is high-expectation and self-directed
  • Comfort in ambiguity and lean operating environments

Strongly Preferred

  • Experience in a DPC, concierge, or cash-pay setting where patient relationships were part of the product
  • Background in functional medicine, integrative psychiatry, or performance-focused care
  • Interest in building clinical workflows and eventually managing a team — prior experience is a plus, not a prerequisite
  • Curiosity about neuroscience, neuromodulation, and the science of brain change

Not Required

  • Prior TMS or brain stimulation experience — we will train you comprehensively

Compensation

Built for builders, not volume.

Benchmarked to the concierge and private-pay tier — not volume-based medicine. First-hire equity commensurate with your role in building this.

The Learning Environment

Train at the frontier of brain health.

You’ll train under Dr. David Carreon — one of the country’s leading neuropsychiatrists, who holds a New York license — under a Collaborative Practice Agreement. New York NPs follow a defined transitional period before achieving full practice authority, giving you a clear, accelerated path to clinical independence. For the right NP, this is one of the most compelling learning environments available right now.
